Why the distinction matters
A record is useful precisely because it does not interpret. When you write down what you observed and when, a clinician can read it and draw their own conclusion. If we told you what an observation meant, we would be guessing about a person we have never met — and we would be replacing the judgement of someone qualified to make it.
